Annual Financial Summary Report

This form must be completed by the club/committee treasurer and general leader at the end of each program year.  The form must be turned in to the MSU Extension 4-H Office no later than November 1.  A copy of each form should be kept with club records.

Club Registration Forms & Information

Macomb County MSU Extension staff needs the information below from its 4-H clubs each year. Clubs must have a minimum of one screened volunteer and five youth from two or more families. Members and screened volunteers enroll online at https://mi.4honline.com. Adults should NOT enroll online until they have completed the Volunteer Selection Process (VSP)

 

New Clubs:

New clubs must turn in the following to Macomb County MSU Extension, Attn: 4-H, 21885 Dunham Road, Suite 12, Clinton Township, MI 48036:

Request for Approval to Establish a Michigan 4-H Group (new clubs only)
4-H Group Tax Exemption Authorization and Request for a Charter (new clubs only)
4-H Club Constitution (new clubs only)
4-H Club Information Sheet
Civil Rights Compliance Statement
Club Officer Information Sheet
Club Roster

Bylaws (Use Bylaws Guidelines, Bylaws Worksheet, and/or Bylaws Worksheet for Cloverbud Clubs as guides)
State 4-H Participation Fee payment ($20 per member, max. $60 per family, paid only once annually, in the county where youth's primary club is
     located. Families unable to pay the State 4-H participation fee may request a scholarship in lieu of payment. Member enrollments will not be approved until participation fees have been paid.)

 

Returning Clubs:

Once club forms are turned in and the club records have been updated, members and volunteers may begin enrolling/re-enrolling online at https://mi.4honline.com. Enrolling for the first time? See the 4-H Online Family Guide for instructions. Re-enrolling for 2017-18? Use Re-enrolling through 4-H Online for guidance. Unable to enroll or re-enroll online? Printed enrollment forms are available from the Macomb County MSU Extension 4-H office by request. Please do not use forms from previous years.

Fairs

Armada Fair: The Armada Fair is August 13-19, 2018. Youth Organization Fair books and forms are available online in May. Your 4-H club leader will submit entry forms to the Armada Fair Office for your entire club. See page 5 of the Armada Fair Youth Organizations Entry Book for fee and deadline information.
 

Michigan State Fair: The Michigan State Fair is August 30-September 3, 2018 at the Suburban Collection Showplace in Novi.

 

Fundraiser Application Form

The Fundraiser Application Form must be submitted to the MSU Extension 4-H office two weeks prior to the event.

Logo & Disclaimer Requirements for Flyers & Other Publications

Logo requirements for 4-H publications have changed!  If you are designing a 4-H brochure, event flyer or other 4-H publication that will be distributed, the following items MUST be included:

These items are required by Michigan State University Extension and Macomb County.  The Macomb County MSU Extension office can not approve or distribute materials without them. The logos and disclaimer are available in different formats through the Macomb County MSU Extension 4-H office.  Direct logo and disclaimer requests to Macomb.4h@macombgov.org or 586-469-6431.

Program Participation Record

Funding for MSU Extension is based, in part, on the programs and activities in which our staff, volunteers and youth participate.  Clubs and committees should use this form as a sign-in sheet for each workshop or training your club or committee does that is educational in nature and falls within a project area.  Include events such as the 4-H Chinese Art Exchange, Macomb County 4-H Craft Workshop, K-9 Karnival, animal clinics, meetings with educational components, etc.  Return completed forms to MSU Extension - Macomb County, Attn:  4-H, 21885 Dunham Rd., Suite 12, Clinton Township, MI  48036

Teen Counselor Program

Michigan State University Extension is accepting applications for High School students for the position of Teen Counselor.  Students will earn certificates of community service hours, while gaining valuable professional work experience in a fun, professional environment mentored by Michigan State University Extension. A letter of reference will also be provided based on performance. This is an excellent opportunity for any student but especially for those considering careers as teachers, youth development, or in the human services sector. For more information, call 586-469-6090.

This form is to be submitted to the Macomb County MSU Extension 4-H Office with the Secretary's Report and minutes after each club meeting.

 

Volunteer Selection Process

The 4-H Volunteer Selection Process (VSP) consists of an application, interview, criminal record check, sexual registry check, three reference checks and an online orientation. Please return your completed application form to MSU Extension, 4hvolunteers@anr.msu.edu. Once your application has been received, you will receive further instructions from MSU. Workshop Registration

You can now access information about and register for State 4-H workshops online.  Registrations will still be accepted traditionally by Macomb County MSU Extension, however, online registration is faster and recommended. Checks should be made out to MSU.

 

Youth Livestock Sale Participation Form

The Armada Fair Youth Livestock Sale Participation Form is required of each 4-H member planning to participate in the 2106 Armada Fair Youth Livestock Sale. Mail or drop off completed forms by the 2nd Friday in January to the Armada Fair Office, 74280 Fair Street, Armada, MI 48005.
